Case Seven - Sclerosing Adenosis

Clinical History: 36 year old female, routine mammogram. Normal physical examination.
Findings:Glandular distortion.
Diagnosis:Sclerosing adenosis.
Discussion: An excisional breast biopsy revealed Sclerosing Adenosis, with florid ductal hyperplasia. No malignancy was found.
Sclerosing Adenosis is a relatively common benign breast lesion, with no known predisposition towards malignancy. The average patient is 30 years old. However, both mammographically and histologically it can have characteristics that resemble carcinoma.
Histologically, hyperplastic proliferation of all elements of the lobule are seen. Dense hyalinization (sclerosis) leads to distortion of the epithelium, which may be mistaken for a malignancy change.
Mammographically, sclerosing adenosis rarely forms a visible mass. It may have clustered microcalcifications, indistinguishable from carcinoma. The differential diagnosis includes radial scar, post-surgical scar, and carcinoma. Biopsy is appropriate approach for diagnosis.
